The Hospice Inpatient Clinical Coordinator (HICC) plays a critical role in supporting hospice admissions and inpatient care delivery. This position is responsible for developing and strengthening relationships with hospital partners, evaluating patients referred for hospice enrollment, and providing General Inpatient (GIP) level hospice care. The HICC conducts comprehensive clinical assessments, facilitates hospice admissions, collaborates closely with interdisciplinary teams, and ensures compliance with all applicable state and federal regulations.
This role combines strong clinical expertise with relationship management and care coordination in a fast-paced inpatient environment.
Build and maintain strong referral relationships with hospital partners to support business continuity and growth
Respond to hospice referrals and obtain Election of Benefits for eligible patients
Conduct comprehensive clinical assessments using Hospice Conditions of Participation, LCDs, and regulatory guidelines
Coordinate symptom management for patients awaiting discharge to home hospice
Serve as the RN member of the Inpatient Hospice Interdisciplinary Team (IDT) for GIP patients
Develop and manage individualized hospice nursing care plans
Participate in weekly IDT meetings and daily GIP huddles
Coordinate transitions for patients no longer meeting inpatient criteria
Provide clinical oversight and hospice education to hospital staff caring for GIP patients
Maintain accurate, timely, and compliant clinical documentation
Graduate of an accredited School of Nursing (BSN preferred)
Minimum of 2 years of experience in Medical-Surgical or Community Health nursing
Hospice or Oncology nursing experience strongly preferred
Active New York State RN license and CPR-BLS certification
Valid NYS Driver’s License preferred
Proficiency with Microsoft Office
Ability to work flexible hours, including weekends as needed
Strong interdisciplinary coordination, communication, and critical-thinking skills
Ability to work compassionately with patients and families from diverse backgrounds
