| Job Summary | This LPN position is a member of the multidisciplinary ambulatory and Transitional Care Management team who is responsible for patient care, patient satisfaction, and clinical outcomes. As an LPN Nurse, you will play a vital role in supporting patients during transitions between healthcare settings, ensuring continuity of care and promoting optimal health outcomes. |
