Job Description

The FGH/Franklin County View Social Worker collaborates with interdisciplinary teams to support individualized patient/resident care, discharge planning, and continuity of services. This role focuses on caring for the whole person by providing psychosocial support, care coordination, and resource guidance to patients, residents, and families.

Key Responsibilities

  • Collaborate with the Patient Services Manager, Nursing Facility Manager, and interdisciplinary team to support patient/resident care and discharge planning.
  • Assess psychosocial, environmental, social, and educational needs throughout the inpatient stay and update plans as conditions change.
  • Provide supportive counseling, education, and assistance with available services and resources, including Medicare, Medicaid, and community services.
  • Develop, implement, and evaluate discharge plans in accordance with Social Work standards and organizational policy.
  • Document assessments, interventions, and outcomes accurately and timely in the medical record.
  • Communicate patient/resident changes effectively to the care team.
  • Meet productivity expectations and maintain required licensure and certification.
  • Participate in performance improvement and professional development activities.
  • Provide coverage for the Case Manager/Utilization Review RN as needed.
  • Perform other duties as assigned by leadership.

Schedule & Work Environment

  • Full‑time position, 40 hours per week, typically 7:30 a.m. - 4:00 p.m.
