Job Summary:

A clinically competent RN who demonstrates expertise in the perioperative field. Effectively delivers care to the surgical patient through the nursing process.

Supervisory Responsibilities:  None

Duties/Responsibilities:

  • Uses only approved abbreviations.
  • Understands patient safety goals & actions taken.
  • Uses 2 patient identifiers before giving treatments, demonstrates process.
  • Understands read back of verbal orders/critical lab values & can state process.
  • Understands procedure to clarify physician orders.
  • Uses standing orders when indicated.
  • Accurately calls report to MD as warranted by condition; reports to other providers as needed.
  • Reports sentinel events or adverse drug reactions or incidents as warranted.
  • Uses final verification before invasive procedures & can verbalize correct process & documents in EHR.
  • Reassesses patient in response to changes in condition after intervention as needed & documents in EHR.
  • Communicates with & provides education to patients, caregivers, & family members in a timely manner (using respect & dignity when dealing with death & dying) & documents in EHR.

Circulator

  • Transports or calls for patient at appropriate time.
  • Review assessment & health status of the patient preoperatively, verifies presence of required documents on record.
  • Assess & provides intervention for each specific patient population i.e. pediatric, adolescent, adult & geriatric.
  • Makes accurate & ongoing assessments of patient’s status & responds appropriately.
  • Plans nursing care setting priorities for nursing action in a logical sequence according to patient needs following established physician preference sheet & direct communication with surgeon & anesthesia.
  • Assists anesthesia during induction.
  • Demonstrates necessary skills & knowledge to provide care for patients in all surgical services.
  • Compassionately gives personal care to provide comfort & well-being to the patient, acknowledging physiological & psychological needs.
  • Provides for appropriate handling & labeling of all specimens, cultures & blood draws according to department policy & procedure.
  • Performs circulating duties according to department policy & role of the circulating nurse.
  • Confirms availability of surgeon before induction of anesthesia.
  • Completes preoperative checklist & actively participates in final time out verification on all procedures.
  • Selects equipment necessary for procedure & assures functionally before start of care.
  • Gives a thorough, concise report when handing off patient care to a relief nurse or at the time of transfer.
  • Communicates immediately with charge person any changes in procedure or patient condition & possible delays.

Provides care, treatment & services

Scrub Nurse

  • As scrub nurse prepares instrumentation & supplies according to physician preference care & scheduled procedure.
  • Demonstrates proper use of instruments, equipment, suturing & stapling devices & other disposable items.
  • Accurately performs all sponge, needle & sharp counts at appropriate times according to policy.
  • Saves & correctly identifies all specimens according to policy.
  • Initiates final verification process prior to start of 100% of procedures.

PACU Nurse

  • In phase one recovery, completes initial assessment of patient’s physiological & psychological status according to policy.
  • Performs ongoing assessment.
  • Develops plan of care & prioritizes interventions based on the patient assessment.
  • Provides for effective pain management & documents response.
  • Gives thorough concise report of all pertinent information when transferring patients to SDS, MS or SCA.

Provides care, treatment & services

  • Demonstrates knowledge of nursing medication administration policy.
  • Verifies correct patient using two patient identifiers prior to medication administration.
  • Checks patient’s allergies before administration of medication.
  • RN obtains, prepares & administers IV’s, RN obtains & administers blood products according to policy & procedure.
  • Observes & documents patient response to pertinent medication.
  • Handles adverse medication & blood reactions in a competent manner.
  • Educates patient on safe use of patient controlled medication administration according to nursing policy.
  • Consistently follows process for securing all medication.
  • RN follows policy for narcotics in the OR.
  • Follows policy for RN administering moderate sedation.
  • Complies with read back policy for telephone orders & reporting of lab results.
  • Demonstrates understanding of & follows policy for use of PPE, hand hygiene, surgical hand scrub & attire in the OR.
  • Demonstrates knowledge of care, disposal & decontamination of all instruments & supplies.
  • Follows appropriate procedure for obtaining, returning & cleaning/disposing of equipment & supplies.
  • Participates in room turnover following policy for cleaning the surgery room after a case.
  • Completes flash sterilization following policy.
  • Performs high level disinfection of all endoscopes.
  • Ensures that supplies & equipment necessary for safe patient care are stored in an organized & efficient manner & available for patient care.
  • Demonstrates correct & safe technique in the use of equipment according to specific product information & policy & procedure manual in the OR/PACU.
  • Demonstrates proper use of the emergency communication system in the OR/SDS dept.
  • Maintains clear, legible variance of hours worked & completes timely.
  • Reviews & updates policies (on request).
  • Participates on teams (on request).
  • Attends 75% of scheduled department meetings & reviews written minutes of those meetings not attended.
  • Attends continuing education to maintain competency.
  • Documentation reflects patient status throughout the perioperative process. Included are patient needs, interventions, patient response & outcomes.
  • Knowledge of OR/PCS modules.
  • Aware of & avoids use of abbreviations on the “do not use list”.
  • Accurately documents use of supplies, implants & equipment for appropriate charging.

Required Skills/Abilities:

  • Strong customer service skills
  • Effectively communicates both verbal & written
  • Able to multi-task, prioritize, interpret & be assertive when necessary.
  • Able to perform basic nursing skills
  • Mechanical ability to deal with a large number of machines & equipment for procedures, i.e. autoclaves, cleaning equipment in central physiologic monitors, patient beds & cart alarm systems.
  • Able to operate basic office equipment.

Qualifications

  • Minimum Associates degree in nursing, Bachelor’s degree preferred.
  • Must be licensed as a Registered Nurse in the state of Indiana; 
  • Minimum of 1 year relevant work experience required.

Physical Requirements:

  • Constant manual dexterity, use of hands & talking; frequent walking, pushing, pulling, reaching & standing; occasional sitting, climbing & stooping.
  • Frequently lifts up to 25 lbs.
