Supervisory Responsibilities:  None

Duties/Responsibilities:

  • Makes home visits for patients of all ages
  • Uses only approved abbreviations
  • Understands patient safety goals & actions taken
  • Uses 2 patient identifiers before giving treatments, demonstrates process
  • Understands read back of verbal orders/critical lab values & can state process
  • Understands procedure to clarify physician orders
  • Uses standing orders when indicated
  • Accurately calls report to MD as warranted by condition; reports to other providers as needed
  • Reports sentinel events or adverse drug reactions or incidents as warranted
  • Uses final verification before invasive procedures & can verbalize correct process & documents in EHR
  • Reassesses patient in response to changes in condition after intervention as needed & documents in EHR
  • Provides treatments (including wound care, venipuncture & IV administration) as ordered in a timely & appropriate manner & documents in EHR
  • Communicates with & provides education to patients, caregivers, & family members in a timely manner (using respect & dignity when dealing with death & dying) & documents in EHR
  • Assessments/evaluations/resumptions are timely, complete & accurate according to federally mandated time frames
  • HIS/HOPE reporting accurate & complete within time frame established by federal government
  • Provides professional assessment (includes nursing, psychosocial & spiritual needs) as well as need for other services & re-evaluates as appropriate
  • Completes, maintains, & updates treatment care plans in a timely manner, identifying all appropriate problems & establishing realistic goals
  • Consideration is given to appropriate resources usage including appropriate visit frequency                                                                                                                         
  • Communicates with physicians & refers to other disciplines as appropriate
  • Participates in case conferences
  • Communicates with nursing facilities as needed
  • Attends IDG meetings; provides daily reports to Patient Care Coordinator
  • Develops aide care plans & oversees visits according to state & federal regulations & documents accordingly
  • Maintains compliance with productivity standards according to departmental requirements
  • Time/visit documentation is accurate & submitted in a timely manner according to departmental requirements & state/federal regulations
  • Discharges are documented within time frames required by state & federal regulations
  • Patient outcomes are equal to or better than available benchmarks
  • Assists with chart audits as needed
  • Demonstrates competencies for all populations served

Required Skills/Abilities:

  • Exceptional communication and listening skills
  • Strong foundation in nursing skills to include: critical thinking, assessment, care planning, & administering treatment
  • Proficient in time management and adaptability
  • Must be able to operate a computer, phone, medical equipment, & automobile

Qualifications

  • Minimum Associates degree in nursing, Bachelor’s degree preferred
  • A minimum of 1 year related work experience required
  • Must be licensed as a Registered Nurse in the state of Indiana

Physical Requirements:

  • Frequent use of hands, walking, sitting, standing, kneeling, reaching, stooping, talking
  • Occasionally may have to lift over 50 lbs
