This is more than a traditional outpatient geriatrics role. The physician will provide comprehensive care for older adults while also helping shape the longer-term direction of St. John s geriatric community care program. The model is designed around the realities of aging not only chronic disease, but also cognition, function, medication burden, caregiver needs, mobility, social circumstances, and access to community resources. The Family Nurse Practitioner provides comprehensive medical services for members of family, regardless of age or sex within scope of training. Services include: Preventative care, immunizations, primary care and treatment, health education, prescribing medications, ordering and interpreting lab testing, physical examinations, STD screenings, vision/hearing, women s health, family planning, and nutrition.
