POSITION SUMMARY/RESPONSIBILITIES The Clinical Pharmacy Specialist Lead is responsible for the provision of comprehensive clinical pharmacy services related to a specific area of practice to optimize safe, efficacious, and cost-effective drug therapy in accordance with all regulatory agencies, including but not limited to The Joint Commission (TJC), Occupational Safety for Health Administration (OSHA), Medicare/Medicaid standards, and Texas State Board of Pharmacy Rules and Regulations. POSITION SUMMARY/RESPONSIBILITIES The Behavioral Health (BH) Social Worker assists Community First members with their Behavioral Health needs by assessing and identifying unmet needs and coordinating care, utilizing Covered Services and community resources with the goal of optimizing the Member's well-being, independence, and integration in the community. The BH Social Worker collaborates with other teams in Population Health Management (PHM) to ensure that the member's physical, behavioral, functional, and psychosocial needs and preferences are met.
