Providers will operate in a team known as the Geriatric PACT (Patient Aligned Care Team), also known as the Geri-PACT. PACT involves veterans working collaboratively with healthcare professionals to plan for their whole-person care and life-long health and wellness, using a team-based approach, led by the geriatric physician. Services include both providing direct patient care and clinical guidance to mid-level providers.
