Provides initial and ongoing participant assessment, care plan development implementation of care plan for participants living in community, congregate, adult home or skilled nursing facility settings. Responsible for full assessment of participants including obtaining health records, performing physical and mental evaluations, evaluating diagnostic tests, and reporting findings and identifying status changes. Performs complete analysis and assessments of participants including recognizing abnormal findings in testing, exercising clinical judgement and interpreting screenings and other diagnostic tests in accordance with medical standards of care. Summary:   Responsible for providing home care nursing assessment and interventions along with care planning and coordination of 24- hour care for to PACE participants as part of the PACE interdisciplinary team. Plans and supervises the personal care and ADL assistance tasks of Geriatric caregivers in the home. Makes visits to participant homes, inpatient facilities and other locations as required to assess participant level of care needs, coordinate with participant, family caregivers, other providers and the IDT.
