SOCIAL WORKER-TRIHEALTH CANCER INSTITUTE: • This position is responsible for working independently within a team framework, while assessing the clinical, environmental, and psychosocial needs of patients diagnosed with cancer and blood disorders within the ambulatory setting. They are part of a multidisciplinary team that must collaborate for purposes of psychosocial needs that arise through course of diagnostics, treatment and survivorship. Collaborates with care teams, including but not limited to hematology, medical oncology, surgical, radiation and neurological. • Demonstrate competency of distress, as determined by NCCN Guidelines and the Distress Screen Tool. Clinician must be skilled in basic biological assessment to determine if there is a need to notify physician (PCP or specialist) of medical changes• Assessment of needs include; financial, medical, social, emotional and familial.
