Health Connect America seeks a Substance Use Disorder Therapist to provide outpatient, in home, and community based counseling to adolescents and adults. In this role, you will complete biopsychosocial assessments, develop person centered treatment plans, and deliver individual, group, and family therapy focused on substance use and co occurring mental health needs. You will collaborate closely with a multidisciplinary team, coordinate with community partners and referral sources, monitor client progress, and maintain timely, accurate clinical documentation while supporting a culture of compassion, integrity, and professional growth.
