Oak Street Health seeks a Nurse Practitioner / Advanced Practice Provider to deliver high-quality primary care to adults on Medicare in underserved communities. In this patient-centered, team-based role, you will conduct comprehensive assessments, manage chronic conditions, provide preventative care, and coordinate with physicians, nurses, and social workers.
