JOB DESCRIPTION: The Certified Diabetes Care and Education Specialist is responsible for the clinical coordination of care related to a specific group of patients or aspect of patient care. Coordination of care extends across the organization and to the outside community. S/he applies specialized knowledge through teaching of patients and family, consultation, research and guidance of other members of the health care team. The organizations Standards of Patient Care and Nursing Practice guide the delivery of nursing services by this individual.
