Home Health Nurse

Interim HealthCare seeks a compassionate 1:1 Home Health Nurse to provide individualized care to patients in their homes. In this role, you'll complete assessments, administer medications and treatments, manage chronic conditions, and deliver skilled nursing services for a defined group of patients. You'll collaborate closely with physicians, therapists, and families to update care plans and promote safety, independence, and comfort. Responsibilities

• Provide 1:1 skilled nursing care to patients in their homes
• Perform comprehensive assessments and ongoing patient monitoring
• Administer medications, treatments, and wound care per orders
• Develop, implement, and update individualized care plans
• Educate patients and families on disease management and safety
• Collaborate with physicians, therapists, and care coordinators
• Document all care accurately in EHR and maintain regulatory compliance
• Monitor changes in condition and report promptly to providers
• Support safe transitions of care and promote patient independence
• Travel to patient homes within assigned service area
