Family Nurse Practitioner

Job Description

Comprehensive Community Health Centers Inc., a Federally Qualified Health Center, seeks a Family Nurse Practitioner to provide comprehensive primary care to diverse, underserved patients in greater Los Angeles. The FNP will perform assessments, diagnose and manage acute and chronic conditions, prescribe medications, order and interpret diagnostics, and deliver preventive and patient education services. Working in a multidisciplinary team, the FNP ensures culturally competent, compassionate, patient-centered care, coordinates referrals, maintains accurate documentation, and supports quality improvement and population health initiatives across multiple clinic locations.

Responsibilities

  • Provide comprehensive primary care to diverse, underserved patient populations
  • Perform histories, physical exams, and health assessments
  • Diagnose and manage acute and chronic health conditions
  • Prescribe medications and therapies in accordance with regulations
  • Order, interpret, and follow up on labs and diagnostic tests
  • Deliver preventive care, counseling, and patient education
  • Collaborate with a multidisciplinary care team to coordinate services
  • Maintain accurate, timely clinical documentation in the EHRParticipate in quality improvement and population health initiatives
  • Support the mission and cultural sensitivity standards of the health center
