Virtual Registered Nurse

LifeBridge Health, Inc. seeks an Onsite Virtual Registered Nurse to deliver high-quality remote patient care from a centralized hub supporting onsite teams. In this role, you will conduct virtual assessments, monitor clinical status, coordinate care, and escalate concerns using EHR and telehealth tools. Responsibilities

• Provide virtual nursing assessments and ongoing monitoring of assigned patients
• Use EHR and telehealth platforms to document care, track status, and communicate updates
• Collaborate with physicians, bedside nurses, and interdisciplinary teams to coordinate care plans
• Triage and escalate changes in patient condition according to clinical protocols
• Support patient and family education on care plans, medications, and follow-up needs
• Contribute to safe discharges and care transitions with accurate, timely handoffs
• Adhere to clinical quality, safety, and infection prevention standards
• Participate in continuous improvement, training, and best-practice sharing within the virtual care team
