Job Description
Healthkeeperz, Inc., a faith-driven, family-owned provider, seeks a CAP Registered Nurse to deliver compassionate home-based care to patients in rural North Carolina. In this role, you will complete comprehensive assessments, develop and implement individualized care plans, provide skilled nursing treatments, manage medications, and educate patients and families. You will collaborate closely with an interdisciplinary team across home health, hospice, and community services, ensuring holistic, dignified care that reflects our mission to serve rural and underserved communities with integrity and respect.
Responsibilities
- Provide skilled nursing care in patient homes, including assessments, treatments, and monitoring.
- Develop, implement, and update individualized care plans based on patient needs and physician orders.
- Administer medications, perform wound care, and manage IV therapies as appropriate.
- Document all care and patient progress accurately in electronic health records (EHR).
- Educate patients and families on disease management, medications, and home care practices.
- Collaborate with an interdisciplinary team across home health, hospice, and community-based services.
- Advocate for patients' physical, emotional, and spiritual needs in alignment with faith-based values.
- Travel to patients' homes in rural and underserved communities, ensuring timely visits and follow-up.
