Job Description
VMSC Emergency Medical Services seeks a Community Paramedic to provide advanced out-of-hospital care, mobile integrated healthcare, and follow-up visits to high-risk patients in Lansdale and surrounding Montgomery County communities. Responsibilities include 911 response support, chronic disease monitoring, medication reconciliation, patient education, and coordination with hospitals, primary care, and social services. The role blends field assessment, telehealth, and in-home visits, emphasizing community outreach, injury/illness prevention, and public education while maintaining accurate documentation and clinical quality standards.
Responsibilities
- Provide advanced life support and community paramedicine services in homes and community settings
- Support 911 emergency response and non-emergency medical transport as needed
- Perform comprehensive patient assessments and monitor chronic conditions
- Conduct medication reconciliation and reinforce care plans from hospitals and clinics
- Deliver patient and family education on disease management, safety, and prevention
- Coordinate care with hospitals, primary care, specialists, and community/social services
- Participate in public education events, outreach, and injury/illness prevention programs
- Maintain accurate electronic patient care reports and documentation
- Follow clinical protocols, safety standards, and quality improvement initiatives
- Engage in ongoing training, skills development, and team-based case review
