Liberty Health is seeking a Home Care Occupational Therapist to deliver person-centered therapy services to adults in their homes. In this role, you will assess functional abilities, develop individualized treatment plans, and implement evidence-based interventions to improve safety, independence, and quality of life. You will collaborate with an interdisciplinary team, educate patients and caregivers, document outcomes, and adjust plans based on progress. Responsibilities
- Provide home-based occupational therapy evaluations and treatments
- Develop and implement individualized, evidence-based treatment plans
- Assess functional abilities, safety, and home environments
- Educate patients and caregivers on exercises, adaptations, and equipment use
- Collaborate with interdisciplinary healthcare and social service teams
- Document assessments, goals, progress notes, and outcomes accurately
- Recommend adaptive equipment and home modifications to improve safety
- Adjust treatment plans based on ongoing evaluation and patient goals
- Ensure compliance with professional, regulatory, and payer standards
- Contribute to quality improvement and outcomes-focused initiatives
