West Cecil Health Center Inc is seeking a Licensed Certified Social Worker to deliver high-quality social work services in a tech-driven healthcare environment. In this hybrid role, you will complete psychosocial assessments, develop and manage treatment plans, and coordinate care using leading software tools. You will collaborate closely with clinicians and product teams, provide crisis intervention and referrals, and leverage data and EHR systems to improve client outcomes. Responsibilities
- Provide licensed social work services to clients using evidence-based practices within a tech-enabled healthcare environment
- Conduct psychosocial assessments, develop treatment plans, and coordinate care with multidisciplinary teams
- Leverage software tools and data platforms to document care, track outcomes, and improve service delivery
- Collaborate with product and IT teams to give feedback on digital mental health and case-management solutions
- Offer crisis intervention, counseling, and referrals to community resources
- Ensure compliance with ethical, legal, and regulatory standards in all client interactions
- Educate clients on digital tools that support well-being and access to services
- Participate in case conferences, team stand-ups, and cross-functional initiatives to enhance integrated care
- Contribute to continuous improvement, data-driven decision-making, and best-practice development
- Mentor junior staff and support training on clinical documentation and technology platforms
