This leadership role involves ensuring the delivery of exceptional end-of-life care, maintaining adherence to the highest standards of patient care, and supporting the emotional needs of our patients and their families. Responsibilities
- Lead and manage a multi-disciplinary team including nurses, therapists, and social workers to ensure quality of care.
- Develop and implement patient care plans and assess ongoing patient and family satisfaction.
- Ensure compliance with all federal, state, and local regulations and adherence to hospice policies and procedures.
- Conduct regular staff meetings and training sessions to ensure team is up-to-date with the latest in palliative care practices.
- Manage budgeting and resource allocation for the hospice program.
- Serve as a key point of contact for family members, providing timely updates and addressing concerns.
- Review and analyze patient outcomes facilitating continuous improvement in care delivery.
