In Home Nursing LPN

Aveanna Healthcare is seeking an In Home Nursing LPN to deliver compassionate, skilled care to pediatric and adult patients in their homes. In this role, you'll follow physician-directed care plans, administer medications and treatments, monitor and document patient status, and communicate changes to the care team. You'll partner closely with families, providing education on disease management and home safety. Responsibilities

• Provide in-home skilled nursing care to pediatric and adult patients under physician orders
• Administer medications, treatments, and wound care following clinical protocols
• Monitor, document, and report patient conditions and changes to the care team
• Collaborate with families, caregivers, and multidisciplinary teams on care plans
• Educate patients and families on disease management, equipment, and home safety
• Maintain accurate clinical records and comply with all regulatory standards
• Use clinical judgment to respond to changes in patient status and escalate as needed
