Job Description

Oak Street Health seeks a compassionate Registered Nurse to deliver primary and chronic care to adult Medicare patients in underserved communities. In this team-based role, you'll perform assessments, develop care plans, administer medications and treatments, and provide education that supports preventative care and improved outcomes. You'll collaborate closely with physicians, care managers, and support staff in a patient-centered, inclusive culture that values innovation and professional growth, helping expand access to quality healthcare where it's needed most.

Responsibilities

  • Provide patient-centered primary and chronic care to adult Medicare patients
  • Perform assessments, develop and update individualized care plans
  • Administer medications, treatments, and vaccinations per protocols
  • Monitor patient status, document accurately in the EMR, and report changes
  • Coordinate with physicians, care managers, and support staff in a team-based model
  • Educate patients and families on disease management and preventative care
  • Support care transitions, follow-up calls, and outreach to high-risk patients
  • Uphold infection control, safety, and quality standards
  • Participate in quality improvement initiatives and population health programs
  • Precept, mentor, and collaborate with colleagues to support a positive culture
