Nurse Practitioner

Oak Street Health seeks a Nurse Practitioner to deliver high-quality primary care to adults on Medicare in underserved communities. In this role, you will provide comprehensive assessments, manage chronic conditions, prescribe medications, and develop individualized care plans with a strong emphasis on prevention. You will collaborate within a multidisciplinary, team-based model to improve patient outcomes and access to care. Responsibilities

• Provide comprehensive primary care to adult Medicare patients, including assessment, diagnosis, and treatment
• Manage chronic diseases such as diabetes, hypertension, COPD, and heart failure
• Prescribe and manage medications in accordance with state and federal regulations
• Develop, implement, and monitor individualized, preventive-focused care plans
• Perform routine screenings, wellness visits, and health risk assessments
• Document all patient encounters accurately in the Electronic Health Record (EHR)
• Collaborate with physicians, nurses, social workers, and care teams to coordinate care
• Educate patients and families on disease management, medications, and healthy lifestyle changes
• Participate in quality improvement and population health initiatives
• Contribute to a patient-centered, inclusive, and collaborative culture aligned with Oak Street Health's mission
