Oak Street Health seeks a Nurse Practitioner to deliver high-quality primary care to adults on Medicare in underserved communities. Responsibilities
- Provide comprehensive primary care to adult Medicare patients, including assessment, diagnosis, and treatment
- Manage chronic diseases such as diabetes, hypertension, COPD, and heart failure
- Prescribe and manage medications in accordance with state and federal regulations
- Develop, implement, and monitor individualized, preventive-focused care plans
- Perform routine screenings, wellness visits, and health risk assessments
- Document all patient encounters accurately in the Electronic Health Record (EHR)
- Collaborate with physicians, nurses, social workers, and care teams to coordinate care
- Educate patients and families on disease management, medications, and healthy lifestyle changes
- Participate in quality improvement and population health initiatives
- Contribute to a patient-centered, inclusive, and collaborative culture aligned with Oak Street Health's mission
