Nurse Practitioner

Job Description

Oak Street Health seeks a Nurse Practitioner to deliver compassionate, high-quality primary care to adult Medicare patients in underserved communities. In this role, you'll provide comprehensive assessments, manage chronic and acute conditions, and develop preventive care plans within a collaborative, team-based model. You'll work closely with physicians, nurses, and care teams to improve patient outcomes, address social determinants of health, and support population health initiatives. Responsibilities

  • Provide comprehensive primary care to adult Medicare patients, including assessment, diagnosis, and treatment
  • Develop individualized, preventive care plans focused on chronic disease management and wellness
  • Collaborate with physicians, nurses, care managers, and social workers in a team-based model
  • Order, interpret, and follow up on diagnostic tests and labs; adjust treatment plans accordingly
  • Educate patients and families on conditions, medications, and lifestyle changes to improve outcomes
  • Document all encounters accurately in the electronic health record (EHR) and maintain regulatory compliance
  • Participate in quality improvement initiatives and population health programs for underserved communities
  • Coordinate referrals to specialists and community resources to address social determinants of health
  • Provide culturally competent, patient-centered care in alignment with Oak Street Health's mission
  • Precept or mentor clinical staff as needed and participate in ongoing professional development
