Kingston Healthcare is seeking a dedicated Registered Nurse to join our Healthcare & Medical Services team at Kingston Health Center of Vermilion. Responsibilities
- Provide direct bedside nursing care to assisted living, rehabilitation, and memory care residents in accordance with physician orders and nursing standards of practice
- Conduct thorough initial and ongoing assessments of residents' physical and psychosocial status and update individualized care plans as conditions change
- Administer medications, treatments, and therapies safely and accurately, following all regulatory and facility protocols
- Monitor residents for changes in condition, perform clinical triage, and promptly notify physicians and other providers of significant findings
- Document assessments, interventions, resident responses, and care plan updates in the Electronic Medical Records system in a timely and accurate manner
- Collaborate with an interdisciplinary team, including physicians, therapists, social workers, and nursing assistants, to coordinate and optimize resident care
- Provide education and support to residents and families regarding diagnoses, medications, treatments, and self-care strategies
- Implement and uphold infection control, fall prevention, and safety protocols to maintain a safe environment for residents and staff
- Participate in quality improvement initiatives, care conferences, and audits aimed at enhancing outcomes and resident satisfaction
- Support, guide, and mentor nursing assistants and other team members, fostering a positive, respectful, and learning-focused culture
- Respond effectively to urgent and emergent situations, utilizing sound clinical judgment and established procedures
