Matrix Medical Network seeks a Nurse Practitioner to deliver high-quality in-home and community-based health assessments for diverse adult populations. Responsibilities
- Conduct in-home and community-based health assessments for diverse adult populations
- Evaluate medical history, perform physical exams, and develop evidence-based care plans
- Collaborate with interdisciplinary teams to optimize patient outcomes and close care gaps
- Document findings accurately in EHR and support quality and compliance initiatives
- Educate patients and caregivers on disease management, medications, and preventive care
- Use telehealth and digital tools to monitor health status and coordinate follow-up care
- Participate in quality improvement projects that support value-based care and cost reduction
