The Home Health Hospice Case Manager functions as a member of the clinical service line team. This Registered nurse is responsible for planning, coordinating, providing, and documenting care for terminally ill patients from day of admission through day of discharge. Utilizing physicians' orders and professional skills, the Case Manager RN develops and implements a home health plan of care that meets each patient's specific needs. The Case Manager acts as a member of the interdisciplinary group team, and reports to the Program Manager. The Home Health Hospice Case Manager directs and leads other assigned team members, such as the Certified Nursing Assistant. Collaborates with multidisciplinary team members to provide age/developmentally appropriate care in accordance with nursing standards of care and practice.
