Position Summary : The Oncology Social Worker (OSW) will provide information and assist oncology and hematology patients, their family and/or caregivers with access to internal and external resources to address practical, financial, and emotional concerns across the continuum of care settings. This includes the management of the distress screening process and referral to appropriate resources to help overcome health care barriers. The OSW will work with the team to get screening completed for all cancer patients at a pivotal medical visit across the continuum of care and contributes toward the creation of screening tools to address psychosocial, social, cultural, and spiritual factors.
