Psychiatric Case Manager (PRN)

JOB SUMMARY: The Psychiatric Case Manager will be responsible for providing support and assistance to patients with mental health issues. This includes assessing patient needs, creating treatment plans, monitoring progress, and coordinating with other healthcare professionals to provide comprehensive care. The Case Manager will work with a diverse population of patients and be committed to the principles of equity, inclusion, and diversity. QUALIFICATIONS: - Bachelor's degree in Social Work or related field is required - At least 2 years of experience in case management or related position - Knowledge of mental health diagnosis, treatment, and community resources - Strong communication and interpersonal skills - Ability to work independently and as part of a team RESPONSIBILITIES: - Conduct assessments with patients to identify their needs and create treatment plans - Monitor and evaluate the progress of patients and adjust treatment plans as necessary - Connect patients with appropriate community resources to meet their needs - Coordinate with other healthcare professionals, such as psychiatrists and therapists, to provide comprehensive care - Maintain accurate and complete records of patient interactions and progress - Communicate effectively with patients, families, and other healthcare professionals - Attend meetings and trainings as required by the company WORKING CONDITIONS: The Psychiatric Case Manager will work in an office setting and may also need to meet with patients in their homes or in the community. The position may require some evening or weekend hours. The Case Manager must be able to travel to different locations within the community as needed.
