Community Health Worker/Promotora PRN We’re looking for a qualified Community Health Worker (Continuum of Care Program) like you to join our Texas Health family. Position Highlights Work location: Home office location – Texas Health Clearfork Medical Office Building in Fort Worth. Travel required across all Texas Health locations. Work hours: PRN “as needed” – Primarily Monday through Friday from 8:00 am to 4:30 pm, however, must be flexible to work required evenings & required weekends This is a grant funded position and subject to the award and continuation of the contract North Texas Healthy Communities, Blue Zones Project Department Highlights Exciting opportunity to work on an award-winning, collaborative community health initiative Opportunity to work daily to improve the health and well-being of the community Work is connected to system changes to improve access to fresh food for all residents Here’s What You Need Education H.S. Diploma or Equivalent Required And Other Successful completion of a Community Health Worker formal training program such as from a community college or other education institution Preferred Experience 6 Months Customer service experience either face-to-face or by phone Required Skills Strong verbal and written communication skills, ability to maintain composure in stressful situations, ability to problem solve, time management skills, detail-oriented, ability to work independently as well as with others, strong organizational & prioritization skills, and ability to motivate and inspire others, ability to complete required reports.
