Job Description

DESCRIPTION OF POSITION: To increase safety and decrease injury of the patient(s) identified at risk for falls and other criteria. The Safety Tele-sitter will be responsible for continuous monitoring of all patients admitted to the Safety Tele-sitter Monitoring program. He/she will also be responsible and accountable for appropriate actions when those patients engage in at risk behaviors.   RESPONSIBILITIES AND DUTIES: Collaborates with unit staff to provide coordinated patient care. Consistently uses scripted communication tools. Demonstrates understanding and proficiency of the use of the AvaSys Telesitter software. Maintains visual observation at all times. Verbally redirects patient over digital 2-way audio device. Immediately summons the nursing staff if the patient requires assistance. Participates in hand-off (IPASS) of pertinent information/behavior when arriving on unit and upon completion of shift. Provides patient(s) with explanations as necessary without counseling or giving opinions. Seeks help or advice as soon as possible when there is a threat to patient(s) safety. Seeks assistance with removal of any potential safety hazard from room by reporting to primary or charge nurse. Demonstrates safe work practices and attitudes, follows saftey rules, works to prevent unsafe conditions and behaviors, and participates in organizational and department safety programs. Participates in collaborative identification and reporting of patient safety issues. Reports unsafe conditions that are observed while monitoring the visual feed to the primary nurse. Participates in the following best practices in a consistent and positive manner; huddles, AIDET, hourly rounding, Service Excellence, and the use of tracking tools and forms, when applicable. EDUCATION AND WORK EXPERIENCE High School Diploma/GED Experience as a nursing assistant preferred. Computer Skills Excellent communication skills; ability to pay attention to detail and respond immediately when there is an observed threat to patient safety. Physical Demands/Work Environment Work requires a variety of physical activities including sitting, standing and walking.  Bending, stooping, and lifting is minimal.  The work environment may be physically confining.  Normal hearing or the use of adaptive hearing devices to enhance hearing required.  Visual acuity required for a variety of activities.  Must be able to sit and utilize PC for long periods of time.  Good manual dexterity required for keyboard activities.  Ability to do shift work as required.
