Job Description

The Bozeman Health Internal Medicine group is seeking a Nurse Practitioner to provide team based primary care to adult and geriatric population of patients. The NP will provide internal medicine management in the outpatient setting to adult patients within the scope of the NP license. NP will provide assessment, diagnosis, treatment and management of acute episodic illness and chronic disease management to internal medicine patients. REQUIREMENTS EDUCATION Required: Masters of Nursing or Doctorate of Nursing Practice, with board certification as Family/ Adult/or Adult Gerontology Nurse Practitioner Preferred: LICENSURE and CERTIFICATION Required: Active Montana state RN and APRN licenses Montana APRN prescriptive authority DEA license Current board certification from ANCC or AANP as FNP/ANP/or AGNP BLS through AHA as healthcare provider Preferred: EXPERIENCE Required: 1-2 years demonstrated competency in the skills and knowledge pertinent to the practice of internal medicine, family practice, or medical subspeciality Preferred: 2 years experience in Internal medicine. KNOWLEDGE OF, SKILLS IN, ABILITY TO, COMPLEXITY AND DIFFICULY: KNOWLEDGE OF: Current with continuing education, including pharmacology, related to MT licensure requirements Personal Computers, hardware and basic software programs including EMR, e-mail, word processing, and spreadsheets. ICD codes and CPT coding –related to office visits and procedures Up to date Knowledge of primary care principles and practices Up to Date Knowledge of internal medicine management guidelines Understanding of primary and specialty services offered through Bozeman Health and regionally and when and how to refer /consult other medical providers Knowledge of diagnostic testing modalities: indications, interpretation and application of labs and imaging Up to date Knowledge of primary care/internal medicine medications: interactions, contraindications, dosing, side effects, administration Knowledge of community resources Knowledge of clinical operations and procedures Procedures and methods of laboratory testing and interpretation SKILLS IN: ABILITY TO: Ability to communicate effectively both orally and written- with colleagues , patients and families Preparation and maintenance of electronic medical record Ability to maintain flexibility to adapt to a variety of work environments and workload assignments Ability to manage stressful situations and to practice self-care Ability to educate and counsel patients and families Ability to provide age appropriate care in a culturally and ethnically sensitive manner Ability to work independently and in a team model of patient care Ability to delegate duties to care team members- to include RNs, MAs, and unlicensed personnel Ability to Follow established BH policy and procedures Ability to evaluate one’s own nursing and advanced practice nursing practice in relation to professional practice standards, guidelines, rules and regulations Ability to attain knowledge and competency as needed for job description. Participates in ongoing educational activities, demonstrating commitment to lifelong learning, seeks experience to maintain skills and competency. Maintains record of educational activities. Ability to maintain therapeutic and professional relationships with patients and peers Ability and commitment to integrate evidence based research into ones professional practice Ability to provide leadership in the practice setting and APRN profession Adhere to the standards of the job description and overall SPIRIT philosophy of BH Provide accountable advanced nursing and medical care to patients, within the Montana scope of practice as an nurse practitioner COMPLEXITY AND DIFFICULTY: NP will work to full scope of practice with MD collaboration as per Bozeman Health bylaws. Patient care situations will range from basic to complex NP must have good self judgement and ability to know their competencies and skill limitations within their scope of practice, and when to refer and consult appropriate medical providers ESSENTIAL FUNCTIONS 95% Time Spent - Utilization of the electronic medical record is expected, keeping accurate records and maintaining compliance with all coding and regulatory bodies. Open charts will be checked every day and sign offs done as indicated. Follow through with patient care such as calling about lab reports and radiology reports is required. Perform health promotion and health maintenance examinations, including health promotion counseling and follow up. Examination, assessment and evaluation of patient with acute and episodic illness including ordering and interpreting labs and diagnostics tests, prescribing medications and therapies, and referring to appropriate medical specialist or for hospital admission. Recognition of patients that are not appropriate for the outpatient setting and appropriate triage and referral is expected. It is expected that the nurse practitioner will have the judgement and ability to consult with other providers within the office and to refer to appropriate providers when the situations arise that are outside the scope of practice and/or individual expertise. The nurse practitioner will be able to work as part of a team as well as independently. They will coordinate work schedules with other providers as necessary to meet the staffing needs of the clinic. The Nurse practitioner will work with medical assistants, dieticians, nurses and physicians to coordinate patient flow. The NP will engage in orientation and training of all new Advanced Practice Providers, as well as nursing and medical students. The nurse practitioner will participate in peer review processes. Examination and evaluation of patients with multiple acute and chronic conditions, including medication review and management, with recommendations to support maximum health and functional status. Meet with patients and families to discuss the patient’s current medical condition, clarify and support goals of care, identify community resources to maximize functional status and quality of life Meet or follow up by telephone or electronic medical record with patients to assess response to medications or treatments, discuss labs or diagnostic test. Coordinate care in-person, by phone, or by fax with skilled nursing and assisted living facilities, families, and caregivers. Communicate effectively and coordinate patient care Document patient examination, assessment, plan of care, response to interventions and follow up in the Electronic Medical Record. 5% Time Spent - Perform all other duties as assigned.
