Job Description

JOB SUMMARY
The Cardiac Rehabilitation RN provides specialized nursing care to patients recovering from cardiac events such as heart attacks, surgeries, or chronic cardiovascular conditions. This role focuses on promoting heart health through patient education, supervised exercise, lifestyle modification, and emotional support.
JOB FUNCTIONS
Job Duties:

  • Assess patients’ physical and emotional readiness for cardiac rehab programs, post surgery and  throughout their outpatient program.
  • Develop individualized care plans in collaboration with physicians, exercise physiologists, and dietitians.
  • Monitor patients during exercise sessions, including vital signs and ECG readings.
  • Educate patients and families on heart-healthy lifestyle changes, medication adherence, and risk factor management.
  • Ensure a safe and supportive environment for patients during all phases of the rehab program.
  • Provide emotional support and counseling to help patients cope with lifestyle changes and recovery.
  • Document patient progress and communicate updates to the interdisciplinary team.
  • Participate in quality improvement initiatives and stay current with best practices in cardiac care.
    Regulatory Knowledge:
  • Complies with regulatory and accrediting agency requirements.
  • Licensed by the Board of Nursing for the state.
